Proper Care Nursing ("we", "our", or "us") is committed to protecting the personal information of our clients, families, and employees. As a provider of home care services, we recognize the importance of maintaining privacy and complying with the Personal Information Protection and Electronic Documents Act (PIPEDA).
This policy outlines how we collect, use, disclose, retain, and protect your personal information, in accordance with the 10 principles of PIPEDA.
1. Accountability
Proper Care Nursing is responsible for all personal information in our possession or under our control. We have designated a Privacy Officer who ensures our compliance with PIPEDA.

2. Identifying Purposes
We collect personal information for purposes including, but not limited to:
Providing safe, appropriate, and personalized home care services
Coordinating care with health care professionals and family members
Managing service delivery, staff scheduling, and quality improvement
Billing, invoicing, and submitting insurance claims
Meeting legal, regulatory, or accreditation requirements
We will always identify the purpose of collection before or at the time of collection.
3. Consent
We collect, use, or disclose personal information only with your knowledge and informed consent, except in limited circumstances as permitted by law (e.g., emergencies, legal reporting requirements).
Consent may be express (written, verbal) or implied (when the purpose is obvious and you voluntarily provide information). You may withdraw your consent at any time, subject to legal or contractual obligations.
4. Limiting Collection
We limit the collection of personal information to what is necessary for the identified purposes. We collect information in a fair and lawful manner, directly from you or with your authorization from third parties such as health care providers or family members.
5. Limiting Use, Disclosure, and Retention
We do not use or disclose your personal information for purposes other than those for which it was collected, unless we have your consent or are required by law.
We retain personal information only as long as necessary to fulfill the identified purposes, comply with legal obligations, or resolve disputes. When no longer needed, we securely destroy or anonymize the information.
6. Accuracy
We take reasonable steps to ensure your personal information is accurate, complete, and up to date. You have the right to request corrections to your information at any time.

We use appropriate security measures to protect personal information from unauthorized access, disclosure, loss, or theft. These include:

You have the right to request access to your personal information. Upon request, we will inform you of the existence, use, and disclosure of your personal information and provide access, subject to exceptions under PIPEDA (e.g., legal privilege, safety concerns).
Requests can be made in writing to our Privacy Officer. We will respond within 30 days.
10. Challenging Compliance
You have the right to challenge our compliance with this policy or PIPEDA by contacting our Privacy Officer. If we are unable to resolve your concerns, you may file a complaint with the Office of the Privacy Commissioner of Canada.
